That's a really really tough one. I don't think it's Johansson, I'll go with Miniussi but have no idea about which event and when.
Re my question on the titles:
Greenleaf got it right: Boggo for the mens with 3 titles and Anne K for the womens. Anne won 2 titles, several of the others reached more finals than her but didn't win more than 1 title.

Where on earth did you get the information from? It's not that hard to find the answers to this kind of question for the men, what with historic rankings available on ATP and steveg, but I don't know anywhere you can get that kind of information for the women.

I think it was 1998 or 1999 aswell. I first started following tennis seriously in 1999 and I can vaguely remember there being at least 4 in the top 200, Sam Smith was still top 100 as she got direct entry to the French before her Wimbledon points dropped off the following month.

I think it was the final of TMS Toronto [as I'm sure that when Roddick won Cincinatti this year, the pundits said that it was the first time he'd won there]. Pretty sure that it was in 2003.
Think the score was 7-6, 6-4 but may be wrong about that
