Yes really. From the ATP rule book, page 89
2) ATP World Tour. If the special exempt places are not needed before the beginning of the qualifying competition, the special exempt places shall be filled by additional direct acceptances selected from the original acceptance list. Thus a player may be pulled out of the qualifying before the first ball is hit and replaced by an alternate list player.
3) ATP Challenger Tour. If the special exempt places are not needed prior to the 12 Noon withdrawal deadline, the special exempt places shall be filled by additional direct acceptances selected from the original acceptance list. If the special exempt places have not been determined at the deadline, and then following the 12 Noon withdrawal deadline it is determined that one or both of the places are not needed, the additional direct acceptance(s) shall be filled by the highest ranked player(s) who signed in for the qualifying of that event, including those who have preserved their eligibility
I can't claim any credit for knowing this as I only looked up the details after someone on this forum corrected me on this matter earlier in the year. Up until then I, like you, assumed that the extra slots always went to players based on the original acceptance list.

That would definitely have been ironic, but you're being misled by the big jump in Wardy's ranking after he won Lexington, which was after the entry deadline here. It's Dustov (the 2nd seed in qualifying) who would have got in - James was actually behind Dan on the alternates list:

3 Barton 217
4 Groth 223
5 Molchanov 230
6 Jones 244 - we think he has a special exempt
7 Evans 247 - we think he has a special exempt
8 Ward 260
So I'm afraid it's all Wardy's own fault for losing to Jones!

Actually Ward would have got in if either Evans or Jones had played and lost yesterday afternoon. In a bizarre twist the people who can get in sometimes changes depending on when the SEs clear. If the SEs clear prior to 12:00 midday on the day of the qualifying sign in then extra DAs go to the the next players on the original acceptance list. If the SEs clear after 12:00 midday then they are replaced by alternates who will be the highest ranked players who sign in for the qualifying draw. This only applies on the challenger tour. On the main ATP tour it is always players from the original acceptance list who replace the SEs.

Btw, there is already one lucky loser place, so this being a Challenger and James being the top seed in qualifying, he only needs to reach the FQR to be certain of a place in the main draw. One of the Q/LL places is against Rochus too ...
